PLEASE APPLY IN PERSON. Applying in person may help speed the time it takes for you to receive your documents and it may save you money. You may complete applications and remit payment to the Jamestown City Clerk, 200 East Third Street, Jamestown, NY 14701. An important piece of identification when traveling to foreign countries, a passport may now be applied for at the City Clerk's Office in the Municipal Building between the hours of 8:30 a.m. and 4 p.m. The City Clerk's Office is a certified Passport Acceptance agency.

Most Passports applied for at the City Clerk's Office are issued and mailed directly to the applicant within 6 weeks.

Requirements:

Proof of U.S. Citizenship

Proof of Identity (photo identification)

Two recent, identical, color passport photographs

Fees:

$100 for those 17 years of age and older
$ 85 for those 16 years of age and younger
Passport Photo - Adult $15.00
Passport Photo - Student $ 7.50

Application for Marriage License

In order to be married in the State of New York, a Marriage License is required and must be applied for at least 24 hours prior the wedding. This must be done no more than 60 days in advance. The marriage license must be used within the State of New York.

Requirements:

Fee of $40

Both parties must be at least 18 years of age

If one or both parties are age 16 or 17, the custodial parent(s) or guardian(s) must appear before the Clerk to sign a consent form. The parent(s)/guardian(s) must present a court order stating that they are soley responsible for the minor, if that is the case.
If one or both of the applicants is age 14 or 15, the parties must present written consent and approval of a justice of the supreme or family court. Parent(s)/Guardian(s) consenting to the marriage of the minor must appear before the clerk. If the affidavit of consent is being made outside the State of New York, it must be notarized and accompanied by a certificate of authentication.

Application for Dog License

All dogs must be licensed by the age of 4 months. Proof of current rabies vaccination and, if applicable, the spay/neuter certificate must be presented at the time of licensing.

Fees:

Spayed/Neutered - $12.50 per dog annually (This includes a local fee of $10)

Unspayed/Unneutered - $20.50 per dog annually (This includes a local fee of $10)

Registration of Business

A Registration of Business is required under the Code of the City of Jamestown for any business operating within the city. This is NOT the same as a business license, which must be obtained through the Chautauqua County Clerk's Office. A Registration of Business includes vital public safety information in case of emergency at the business being registered.

Requirements:

There is no fee associated with this registration.

Application to Conduct an Auction

Auctions may be conducted in the City of Jamestown with the proper approval and completion of the application.

Requirements:

Fee of $50

Surety Bond of $5,000

Application for Hawkers, Peddlers and Solicitors License

All Hawkers, Peddlers and Solicitors must be licensed and fall under the laws and provisions of the City of Jamestown.

Requirements:

Fee is $500 except for honorably discharged veteran's residing in Chautauqua County or those engaged in Interstate Commerce.

Cash Bond or Surety Bond of $5,000 may be necessary in some cases.

Application for Pawn Broker, Second Hand Dealer or Junk Dealer

Requirements:

Fee of $125

Pawn Broker, Second Hand Dealers also requires a State License and a Surety Bond of $5,000.

Application for Mechanical Device License

Requirements:

Fee is $50 per machine

Additional inspection or re-inspection fee of $25

Application for Class 1/ Class 2 Electrician's Examination

Requirements:

Fee is $30

Application must be on file for 14 days prior to examination.

After successfully passing the exam, a Class 1 License is $300, or $310 if late fee is applicable. A Class 2 License is $100, or $110 if late fee is applicable.

Each license requires a passport-size ID photo, which may be obtained in the Clerk's Office for $15.00.

Application for Solid Waste Hauler Vehicle License

Requirements:

Fees of $125 plus $35 per additional vehicle

If planning to collect solid waste from the city's right-of-way, a certificate of insurance naming the City of Jamestown as loss payee and as an additional insured must accompany the application. The certificate must be in a form acceptable to the City of Jamestown's Corporation Counsel. The minimum amount of coverage must be $100,000 per single occurrence and $300,000 per multiple occurrences.

Application for Taxicab Driver's License

All persons wishing to become a licensed taxicab driver in the City of Jamestown must obtain this license in addition to a valid Class E or higher New York State Driver's License.

Requirements:

Fee of $20

Two passport-sized photographs which may be obtained in the City Clerk's Office for $15.

Application for Taxicab Vehicle License

All taxicab vehicles in the City of Jamestown must be licensed independently of their drivers.

Requirements:

Fee of $75 per vehicle

Application for Taxi Rate Card

A Taxi rate card must be displayed in properly licensed taxi cab vehicles where the card can be easily seen by customers at all times.

Requirements:

Any change in rates must be approved by the Public Safety Committee

Application for 911 Towing Rate Card

Anyone providing emergency towing service thru 911 Dispatch must have a 911 Towing Rate Card displayed in the towing vehicle where it can be easily seen at all times by customers.

Requirements:

Any change in rates must be approved by the Public Safety Committee.
